Melasma Treatment in Pakistan: What Actually Works on Dark Patches
Quick answer: Melasma is brown or grey-brown patches that appear on both sides of the face. Daily sunscreen is the main treatment. Add vitamin C in the morning and glycolic acid or glutathione at night. Expect softer edges by week 4, a clear difference by week 8, and your best home result by weeks 12 to 16.
If you have brown or grey-brown patches spreading across your cheeks, forehead or upper lip, and they sit in roughly the same place on both sides of your face, you are probably dealing with melasma. It is one of the most searched skin problems in Pakistan, and also one of the most badly treated.
This guide covers what actually helps, how long it honestly takes, and the products that make melasma worse even though they look like they are working at first.
Is melasma the same as dark spots?
Most people use "dark spots on face" and "melasma" interchangeably. The treatment is different, so it is worth two minutes to work out which one you have.
| Melasma | Post-acne dark spots | |
|---|---|---|
| Shape | Large patches with soft, blurry edges | Small and round, exactly where a pimple was |
| Where | Cheeks, forehead, upper lip, jawline | Anywhere you broke out |
| Symmetry | Usually mirrored on both sides | Random |
| Timeline | Months to years, and it comes back | Fades in 2 to 4 months |

What causes melasma on the face
Four things drive it, and three of them are in your control.
Sun exposure. This is the big one. Pakistan's UV index touches 10 to 11 in summer, roughly double what most of Europe gets. UVA passes through glass and cloud, which is why so many people here have heavier pigmentation on the driver's side of the face. You are getting a dose every time you sit in traffic.
Hormones. Pregnancy, birth control pills and thyroid problems all trigger it. That is why melasma is sometimes called the pregnancy mask. Hormones play a part in about one in four affected women (DermNet: melasma).
Heat on its own. Not just UV. Standing over a stove, ironing clothes, long hot showers. Heat activates pigment cells directly, which is one reason melasma is harder to control in Karachi and Multan summers than it is in Murree.
Anything that irritates the skin. Bleach, waxing, harsh scrubs, lemon juice, toothpaste, baking soda. Every episode of inflammation tells your skin to produce more pigment. Kitchen remedies are the fastest way to turn a light patch into a dark one.
Why is sunscreen the main melasma treatment?
This is where most melasma budgets get wasted. You can buy the most expensive pigmentation serum on the shelf, but without daily sun protection the patches come straight back. Four weeks of serum can be undone by one uncovered afternoon.
What actually matters:
- Wear it every day, including winter, and including indoors if you sit near a window.
- SPF 30 is the floor. For melasma, go higher. SPF 60+ Sunblock gives you more margin for the amount most people under-apply.
- Use two finger-lengths for face and neck. Almost everyone uses about a quarter of that, then concludes sunscreen does not work.
- Reapply every two to three hours if you are outdoors.
- Hats, umbrellas and car window film work with sunscreen, not instead of it.
If you can only afford one product for melasma, buy the sunscreen. Everything else is secondary and we would rather tell you that than sell you a serum that will not hold.

The ingredients that genuinely help
Vitamin C, in the morning
Vitamin C interrupts pigment production and evens out tone over time. It also boosts what your sunscreen is doing, so the two belong together. Apply Vitamin C Serum after cleansing and before sunscreen.
Be realistic about what it does. It lightens existing patches and slows new ones. It does not erase melasma. First visible change usually lands somewhere in week four to six.

Glutathione
Glutathione works further up the same pigment pathway, which makes it a sensible partner to vitamin C. Glutathione Serum applied at night is a reasonable addition once your sunscreen habit is solid.
One straight warning, because it matters here. Glutathione injections and drips are sold all over Pakistan for skin lightening. The evidence behind those claims is weak, and injectables carry real risk. We make the topical serum and we do not recommend the injections.

Glycolic acid
Glycolic 7% Toning Solution lifts the older pigmented surface cells so the fresher skin underneath shows through. Two or three nights a week is plenty.
Two rules. Do not use glycolic and retinol on the same night. And be stricter about sunscreen the next morning, because freshly exfoliated skin burns faster.
Niacinamide
Niacinamide calms inflammation and blocks pigment from being handed off to surface skin cells. It is one of the most forgiving actives available, and it plays well with everything. BB Cream SPF 15 with Niacinamide combines it with light coverage and some sun protection, which is useful on days when you are not doing a full routine.

What does not work for melasma, and what can harm?
Skin-lightening creams with no ingredient list. Skin-lightening creams sold in many countries, including online, have been found to contain mercury (World Health Organization: mercury and health) or undeclared steroids. They work impressively for two or three weeks. Then the skin thins, red vessels show through, fine hair grows on the face, and the moment you stop, the pigmentation returns darker than it started. That rebound takes months to repair. If a cream has no ingredient panel and no manufacturer address, do not put it on your face.
Lemon juice, toothpaste, baking soda, aggressive ubtan scrubs. These damage your skin barrier. Damaged barrier means inflammation, and inflammation means more pigment. You are feeding the problem.
Frequent bleaching and upper lip waxing. Upper lip melasma is very often waxing irritation that never settled. Threading is gentler.
Any product promising a change of skin colour in seven days. Skin renews itself over weeks, not days. Nothing that changes your colour in a week is repairing skin. It is doing something else, usually with a steroid.
How long does melasma treatment take?
These are the numbers no advertisement will give you.
| Time | What to expect |
|---|---|
| Weeks 1 to 2 | Nothing visible. Skin may feel better hydrated. That is all. |
| Week 4 | Edges of patches start to soften. New pigmentation slows down. |
| Week 8 | Clear difference. Lighter patches much reduced, deeper ones still there. |
| Weeks 12 to 16 | Best result you will get at home. Deep melasma still will not clear completely. |
| Ongoing | Sunscreen for life. Stop it and the patches return. |
What is a simple melasma routine?
| Morning | Night |
|---|---|
| 1. Gentle cleanser | 1. Gentle cleanser |
| 2. Vitamin C Serum | 2. Glycolic (2 to 3 nights) or Glutathione Serum |
| 3. Moisturiser | 3. Moisturiser |
| 4. SPF 60+, without exception | — |
Four steps. If someone is selling you seven products for melasma, they are filling a basket, not treating your skin.
When should you see a dermatologist for melasma?
- The patches are spreading quickly or changing colour
- You previously used an unlabelled skin-lightening cream and your skin is now thin or red
- Four months of a consistent routine has produced no change at all
- You are pregnant, since several actives are restricted and you need supervision
A dermatologist has options you cannot buy over the counter, including oral tranexamic acid and professional peels (American Academy of Dermatology: melasma treatment). For deep, long-standing melasma that route is usually the right one.

Common questions
Can melasma go away completely?
Usually not. It can be controlled and made much lighter. It tends to return if sun protection stops.
Will pregnancy melasma go away by itself?
Sometimes, within six to twelve months after delivery, but only if you stay protected from sun in the meantime.
Do I really need sunscreen in winter in Pakistan?
Yes. UVA levels stay fairly constant year round, and UVA is what drives pigmentation.
Can I use vitamin C and glutathione together?
You can, but you do not need to. Start with one, give it six to eight weeks, then decide.
What is the minimum I should spend?
Sunscreen delivers the most benefit per rupee for melasma. See the current price of our SPF 60+ Sunblock. Buy that first, add a serum later.
